Donna,

Have you seen how far the jaundice has spread? Is it in the extremities? Rising jaundice with a lethargic baby and poor latch to boot is a very danergous combination esp with parents unwilling to try and correct even the frequency of feeds. Is the latch better now or does she still have painful and cracked nipples? If parents are not willing to follow your advice you might have to refer them elsewhere explaining exactly why you can no longer be their LC. This baby could be in very serious danger right now! Have you spoken with the doctor of this baby? Have you shown or explained to them that a wet diaper needs to be about 3 tablespoons worth of urine? There are also pictures of what poopy diapers should look like I think it is in Barbara's book. What color are the stools at this point? It is very trying to say the least to work with parents who say they want help but aren't willing to actually follow your suggestions. But from the sounds of it this baby could be in imminent danger. I hope you are able to convince them today they need to start feeding this baby!

Warmly,
Donna Goggin-Dolwick, IBCLC
