This can be a good tool, as you said, for getting older babies to take the
breast.  Often in adoption situations or relactation, this is a way to teach
the baby that nursing is feeding, usually using a lact-aid tube threaded
through the nipple hole so the child is getting milk while being held in the
nursing position, and still has that familiar silicone nipple while in
transition.

As a regular nipple shield, I can't see how it would work.  If there is a
milk supply, can we expect the breast to be stimulated enough through the
rubber to cause a letdown and actually supply milk?  I absolutely can't see
it working in any situation.  Plus, as someone said, it surely couldn't be
safe.
