I think this has been discussed before, but Chicago Insurance Company
covers malpractice for LCs.  On the renewal notice I just received, it
says, for all residents except Ohio-

The premiums are based on
full-time (20 hrs/wk or more)
$410 for $2,000,000 per incident, $4,000,000 aggregate
$350 for $1,000,000 per incident, $3,000,000 aggregate

part-time (less than 20 hrs/wk)
1/2 of the above amounts
