I've had a mom with this same scenario.  She continued to have the bleb (AKA
nipple blister), and continued to have to have it lanced throughout her
nursing relationship.  I don't know whether the skin grows tougher, or what
the cause is.  This mom also added lecithin to her diet, with no noticeable
improvement.

Having had two of them personally, I found them extremely painful.  The
relief upon lancing it is immediate.  Doing breast compression to extract
the 'backed up' milk upon lancing was also very helpful.  Some moms find
frequent nursing helps prevent these.
