I noticed in your description you stated "she clicks at the breast
and on the bottle"  Perhaps baby is trying to make the breast feel
like the bottle?  No mention was made of why or how often bottle is
being used, but perhaps another alternate feeding method if it is
necessary for some reason.
(I know this is going to stir up controversy, but here goes anyway)
The behavior you describe is what I have seen after use of the NUK
or "Orthodontic" nipple.  While some LNers have said it is the style
they prefer if an artificial nipple must be used, I have to
disagree.  When I see a mom with a nipple that looks like the NUK I
know there is a latch problem!  I suspect it can teach tongue
patterns we don't want transferred to breast (in addition to all the
other possible drawbacks to pacifiers in general).
It is my understanding that when the NUK was first developed it had
a shank about an inch to an inch and a half long after the nipple
portion and before the bottle cap.  Perhaps this did encourage a
tongue placement closer to nursing, but apparantly when the patent
was sold off, the "shank" was eliminated (to save a few pennies?)
and now the positioning in baby's mouth in entirely different.  The
other story I heard was that the shape was determined by taking a
quick mold of a mom's nipple after baby released it.  Ouch!  and not
a reliable image since the shape changes so quickly after it leaves
baby's mouth.
