Don't know where she heard it but one of my early BF mentors when I was a new mother, described the transient feeling she occasionally got at let-down by saying it was like having a length of string looped around each nipple and then someone pulling outward on the ends.  Many women nod when they hear this description.  I have always figured it could be the erectile tissue under the areola waking up against the pressure of a distended breast, as the sensation seems to be most marked if the mother is feeling a bit engorged and it subsides quickly when the milk starts flowing.
Right now I am just shaking my head at the thread about who is able to touch other people's breasts in various state jurisdictions.  Can you still get a shoe salesperson to squeeze your foot to check for a proper fit?  I guess we have a more relaxed attitude to bodies here.  Whew!  We all need to respect the integrity of clients' bodies, of course, but it is easy to work with women who see their lactating breasts as something almost public.
