Here are the possibilities that come to mind:
Overactive letdown sounds reasonable since baby does not do it on bottled
ebm.....but....
Positioning, latch, seal on breast, swallowing issues etc need to be looked
at since mom bf once per day and mostly bottlefeeds, may not be positioning
well, may not be able to breathe, swallow whatever at breast. Needs feedings
observed.
Fit of mother's breasts/baby's mouth - maybe nipple makes him gag, are her
nipples large, long? Again, direct observation is needed.
Maybe baby has tongue tie or other such issue and can't handle the letdown,
but can bottlefeed, who knows?
Or could be reflux, as the description of after the feed sounds like it
could be this. Maybe the positioning is different with breastfeeds and less
reflux with bottles? Maybe try football hold?
Well, good luck, there are several possibilities.
