Kristin,

This example serves to illustrate several points.  A rocking chair is not
always a good place to nurse, especially if one is prone to hypoglycemic
faints.  Diabetics do need individualized care, especially during pregnancy
and lactation, as these are constantly evolving states.    I'm picturing a
busy new mom, who maybe forgot her protein/carbohydrate snack before going
to sleep or maybe isn't adjusting her insulin doses appropriately or is
still eating like she used to and I'm hoping that breastfeeding isn't blamed
for the problem.

Breastfeeding does require a change in management for the diabetic mother.
Her insulin needs will usually decrease while her energy needs go up.  This
will vary according to the stage of lactation.  If proper adjustments are
not made this sort of disaster may occur.  If the mom is very brittle in her
control I'd be inclined to set up a sort of nursing safe spot where baby
would not fall on the floor if mom passed out (pillows supporting arms, feet
up on a footstool - picture a pillow "nest" or use a sling).  Best solution
of course is better control while continuing to breastfeed.

Ruth Lawrence,s "Breastfeeding A Guide for the Medical Profession" has an
excellent section on diabetes and lactation on pages 515 -521 of the fifth
edition.  Any medical library should have this reference.
