I will be commencing a prenatal breastfeeding education program at a
WIC office in the New York City region.  In order to assess the
effectiveness of the program, I want to hand out a pre-course and
post-course evaluation form.  The 3 areas which I want to assess are:

1. confidence in the ability to breastfeed
2. strength of the conviction/decision to breastfeed
3. knowledge (some basic areas)

Does anyone out 'there' have or know of such a tool (even if it is
just one or two of the 3 areas mentioned above, that would be great).
 If you have a reference, that would also be helpful.  Anything would
be appreciated.
