I have a couple of thoughts on this situation (5wk old, non-latching, mom in
pain, pumping and bottle feeding).  One is the very real possibility that
this mother is a sexual abuse survivor.  Kathleen Kendall-Tackett wrote an
excellent article on this for LLL's Leader magazine, Leaven in April/May
1997.  I believe this article is also online at LLLI website
<http://www.lalecheleague.org>.  The fact that the use of a nipple shield
didn't decrease the pain leads me to believe that previous sexual abuse is
possible.  Mom apparently did not report pain with pumping ... It may be that
this mother would rather stop breastfeeding than delve into her personal
history with lactation consultants and counselors, so be prepared for that.
It could be that the mother does not want to breastfeed but only appears to
want to breastfeed, who knows?
  The fact that the baby does strange things on the breast when latched on
doesn't surprise me given the fact that he has been bottle-feeding from
pretty much day one of life.  He may like being at the breast, skin-to-skin,
next to his mommy, but he has no clue about the mechanism needed to extract
milk.  He can chomp on and slide down on the bottle nipple, not hurt anyone
and *still get milk*.  Why wouldn't he do the same at the breast (asked
rhetorically)?  The mom's nipple blanching can be a result of this baby's
bottle-feeding behaviors, transferred to the breast.
